Direct-to-Implant Reconstruction

Direct-to-implant reconstruction allows breast implants to be placed at the time of mastectomy without the use of tissue expanders. Breast Reconstruction Without Implants

For patients who do not want implants, reconstruction without implants is an option. These approaches use your own tissue rather than synthetic materials.

SWIM Flap

The SWIM Flap is a technique that uses tissue from the lower abdomen to reconstruct the breast while preserving muscle. The SWIM Flap was invented by Dr. Lisa Cassileth, who developed this advanced approach to provide natural reconstruction. This approach may offer a natural look and feel while avoiding implants. As the originator of the SWIM Flap technique, Dr. Cassileth is qualified to perform this procedure, particularly in complex or previously treated cases. Not every patient is a candidate, and eligibility depends on anatomy, prior surgeries, and cancer treatment history.

 

Fat Transfer

Fat transfer-only reconstruction uses purified fat from another area of the body to restore volume. While this approach can be effective for select patients, it has limitations and may require multiple stages. Fat transfer is often best suited for patients with smaller volume needs or as a refinement technique after other reconstruction methods.

Reconstruction without implants may be appropriate for patients who prefer autologous options, have concerns about implants, or have medical considerations that make implant-based reconstruction less suitable. A detailed consultation is essential to determine candidacy. All options are discussed within the context of long-term breast health and cancer care.

Reconstruction After Lumpectomy or Mastectomy

After a lumpectomy, reconstruction may involve reshaping the breast or restoring contour changes caused by tissue removal. In some cases, reconstruction is performed at the same time as cancer surgery, while in others it is planned later.

After a mastectomy, reconstruction can begin immediately or be delayed until cancer treatment is complete. Patients often have flexibility in timing, and choosing to wait does not eliminate future reconstruction options. What matters most is aligning reconstruction with cancer care priorities and your personal preferences.
